Enroll in the Plan
Basic Life Insurance
You are automatically enrolled in the Basic Life Insurance plan as of your date of hire or the date you become a Benefits-Eligible Employee.
Supplemental Life Insurance
You may enroll in Supplemental Life Insurance:
- Within 31 days of your date of hire or the date that you become a Benefits-Eligible Employee by visiting Employee Self Service.
- Any time during the year by submitting the Benefits Change Form and submitting it to the Benefits Office. Coverage is not automatic, you must demonstrate proof of insurability by completing Prudential's Evidence of Insurability process. Prudential will contact you with information regarding the Evidence of Insurability process and must approve your application before your increased level of coverage takes effect.
Designating Beneficiaries
Beneficiaries can be designated:
- During the initial enrollment for basic or supplemental life insurance.
- Any time during the year by submitting the Change of Beneficiary Form.
