Change of Address

As soon as you have your new address:

  • Log-on to Employee Self-Service using your cnet ID and password to update your address.
  • To change your address with the University of Chicago Health Plan, contact Benefits at Benefits@uchicago.edu.
  • If you move to an area where your HMO does not have providers, you may need to change your medical plan within 31 days of your move.
  • To change your Primary Care Provider, contact  your medical plan.
  • Visit CONEXIS to change your elections for the Qualified Transportation Program.
  • Contact TIAA-CREF at 800.842.2776 and/ or Vanguard at 800.523.1188 to change your address on your retirement plans.
